The FPSC and the Ministry of Health are continuing to collaborate and support all PCN Wave 1 and 2 communities to lead the implementation, spread and sustainability of team-based care within the context of Patient Medical Homes and Primary Care Networks. This document provides the guidelines around funding."

The FPSC and the Ministry of Health are continuing to collaborate and support all PCN Wave 1 and 2 communities to lead the implementation, spread and sustainability of team-based care within the context of Patient Medical Homes and Primary Care Networks. "These guidelines are intended to offer clarity about information sharing between GPs and community partners for Mental Health and Substance Use (MHSU) adult patients (non-urgent care).
They were prepared to exist within the context of current legislation, including two privacy laws:
