The PMH Engagement Journeys were designed to capture eight divisions' early implementation of the patient medical home (PMH) model of health care. The stories offer a look at the challenges and successes the divisions experienced along their journeys, and the resources they created to engage members and partners around primary care. It is the hope that these stories will act as a resource, support, and source of inspiration for other divisions beginning their journeys to an integrated system of care via the PMH."
