-Facilitate learning and improvement within the collaborative structures on Vancouver Island, and

A discussion of the conditions for success is included. As part of the General Practice Service Committee’s (GPSC) ongoing evaluation of Patient Medical Home (PMH)initiatives in BC, this case study helps to build a provincial picture of PMH innovation and implementation."
